    We will have our first virtual discussion for the SIG on tarlatamab on Wednesday 8/12 at 11am PT / 12pm MT / 1pm CT / 2 pm ET to try and capture as many folks as possible around the lunch hour. Also posted this as an announcement, and just posting here as well! Discussion points will include outpatient monitoring protocols, instituting prophylactic tocilizumab, and maintenance with IO. Virtual meeting link

    Appears I missed a good topic of discussion.  I am interested in hearing from other facilities where tarlatamab is given outpatient in an oncology office and then observed in a nearby, non-integrated facility.   If any facilities who are currently doing this would be able to share how its going I'd love to hear from them.

    Thank you to everyone who was able to participate last week, and we missed those of you who couldn't make it! I have attached a (Teams generated) transcription of the discussion - please note there may be some errors as I did not edit it. :) 

    Key takeaways from the discussion:

    1. Outpatient monitoring remains challenging - most centers on the call continue to use the 22-24 hour IP observation. Centers considering or working toward OP monitoring are typically incorporating eligibility criteria to select for appropriate patients.
    2. CRS prevention strategies - most centers using "pill-in-pocket" dexamethasone; some discussion on post-infusion dexamethasone. Prophylactic tocilizumab not robustly supported by data/guidelines, so reimbursement remains a concern. 
    3. Off-label and non-SCLC uses - various centers have treated patients with small cell GU cancers, NETs, and medullary thyroid carcinoma. Discussion about incidence of CRS/ICANS and difference seen in the non-SCLC patients.
    4. Maintenance tarlatamab not widely embraced quite yet - generally waiting for the DeLLphi-305 data to be presented/published. Discussed concerns regarding the impact on bed availability, infusion chair capacity, monitoring resources, and overall patient volume.
    5. Lurbinectedin and IMforte - discussion on pending label removal of second-line indication and limited use of the IMforte maintenance regimen, and if this may affect treatment algorithms with anticipated DeLLphi-305 data.
